Patient First Hospice & Palliative Care is growing, and we're looking for a compassionate PRN Certified Nurse Assistant (CNA) to join our team and serve Houston and the surrounding areas .

Position Overview: PRN Certified Nurse Assistant (CNA) :

  • The Certified Nurse Aide and Homemaker Services Coordinator (CHSC) is responsible for the day-to-day coordination, supervision, and evaluation of Certified Nurse Aides (CNAs) and Homemakers to ensure the delivery of safe, compassionate, and high-quality patient care. Working under the direction of the Director of Nursing, the CHSC supports the effective implementation of patient care plans and promotes consistency, accountability, and compliance with agency policies, procedures, and applicable federal and state regulations.
  • The Hospice Aide (CNA) assists with the orientation, training, competency assessment, scheduling, and performance management of CNA and Homemaker staff. The position serves as a key liaison between aide staff, patients and families, and the interdisciplinary team, addressing staffing needs, patient care concerns, and service-related issues in a timely and professional manner.
  • The Hospice Aide (CNA) may provide hands-on support in the field as needed and is expected to model patient-centered care, professionalism, dignity, respect, and compassion. This position plays an essential role in promoting patient safety and comfort while supporting the mission and values of Patient First Hospice & Palliative Care.

Education & Licensure

  • High School Diploma or GED required.
  • Must maintain a current, valid Certified Nursing Assistant (CNA) certification or Licensed Vocational Nurse (LVN) license, as applicable, in accordance with applicable state and federal requirements.
  • Must meet all applicable regulatory and agency requirements for the position.

Experience

  • Minimum of two (2) years of clinical experience as a CNA or LVN.
  • At least one (1) year of experience in hospice, home health, long-term care, or a related healthcare setting preferred.
  • Previous supervisory, leadership, coordination, or staff mentoring experience preferred.
  • Experience with hospice aide services, home-based patient care, or interdisciplinary team coordination is highly desirable.

Knowledge, Skills & Abilities

  • Demonstrated ability to lead, coach, mentor, and support CNA and Homemaker staff.
  • Strong verbal and written communication skills with excellent attention to detail.
  • Ability to effectively prioritize, organize, delegate, and follow through on multiple responsibilities.
  • Strong time-management and problem-solving skills.
  • Ability to review clinical documentation for accuracy, completeness, and timeliness.
  • Working knowledge of infection prevention, safety practices, standard precautions, patient confidentiality, and HIPAA requirements.
  • Proficiency with basic computer applications and electronic health records/documentation systems.
  • Ability to communicate effectively and professionally with patients, families, caregivers, clinical staff, and leadership.
  • Compassionate, dependable, self-motivated, and committed to providing patient-centered care.
  • Ability to work independently while exercising sound judgment and knowing when to escalate concerns to the Director of Nursing.
  • Ability to remain calm, professional, and effective when responding to urgent, sensitive, or high-stress situations.
